Driver (independent contractor)
Complete Form W-9 so BWT LLC can report your contractor payments.
Employee
Complete the federal W-4 and Massachusetts M-4 withholding forms (one form, one signature). Then complete Form I-9 on your first day.
What to do
- Open the form for your role above, fill in every line, tick the certification, and type your full legal name as your signature.
- Press Submit. Your form is sent securely to the BWT office; nothing needs to be printed or emailed.
- Employees only — Form I-9: complete Section 1 on or before your first day of work and bring original identity and work-authorization documents (see the list on the form) no later than your third business day. Download Form I-9.
- If anything changes later (address, filing status, dependents), just submit a new form.
