Employee Withholding (W-4 & M-4)

Please fill the following documents

New BWT LLC employees complete the federal W-4 and the Massachusetts M-4 so we withhold the right amount of tax. Both are on the form below and are signed together. Questions: bwtansport@bwtllc.net.
